Becoming an approved broker with Liberty Debt Care is quick and easy. Just fill out our broker approval application and fax it in to the number below. We can get you approved in 24 hours. And while your approval is being processed, you'll still be able to get your applications in for approval. Here's how the process works:
Step 1:
Download and Complete the Broker Application Package and W-9 form:
The following items are required to be submitted along with the above documents:

Copy of Articles of Incorporation/Organization /Partnership documents

Resumes of all owners/principals of company

Copy of all applicable broker licenses and exemption letters

Copy of Driver License

Tax ID or W9
Errors & omission insurance, fidelity bond or surety bond (if applicable)
Already approved and need to add another branch location ? Download and complete the Additional Branch Application.
Step 2:
Enclose all applicable documentation that pertains to your company along with completed Broker package and return the entire package to our corporate headquarters as follows:
